Looks like this could do what I am looking for, but everything defaults back to the lead_follow_up.php sign in page?

If you mean "login page", then that's a symptom of PHP session failure. The script could not save your session state and you got kicked out to the login page, being treated as "Time Out." Please ask your web administrator to fix PHP session.
