Is it possible to have one html page comprising of approx 12 separate headings with a choice of 4 or 5 options to vote?

I've tried coping pasting etc but with no success.

I'm not sure if I exactly understand what you meant by "12 separate headings" but If you mean "12 separate polls in one html page", the answer is yes, you can have it.
