Please help me to implement multiple questions, if the user submit the first question it will appear the result, and when the user hit the next button it will display the next questions and so on. I really need this.

You can create 5 polls and link them together. Open the template for a result page ( web/(poll-name)/tpl.result.inc.php ) and put a link to the next poll on it.
