This is a free "Life Insurance Quotes Request Form" PHP script with a back-end database.
The script allows visitors to fill a "Life Insurance Quotes Request Form" and sends
the form data to a web site administrator. The script also saves the form data
in a MySQL database so the administrator can search, view, edit, and delete
the forms from a web browser.
