Very nice script! I love it! I Would Like to know if it's possible to make the form not accessible to everyone, a.k.a only the admins who have access to login page can use the form. I would like to setup a purchase order form, hence i only want just admins to be able to access it. Thanks before

That's an odd request! Unfortunately, there isn't a turn-key solution for it. You just have to dig into the code and modify it. Sorry about that.
